Meeting of January 26, 2010
The Human Services Board of Wabasha County, Minnesota, convened in Regular Session at the County Services Building, in the City of Wabasha, Minnesota at 9:00 a.m., January 26, 2010 in accordance with MS 402.01 to 402.10. The previous meeting was recessed to this time and place. The following members were present: Commissioners Dwelle, Hall, Norman, Riester, and Windhorst Absent: None
DWELLE – RIESTER
BE IT RESOLVED, that the Wabasha County Board of Commissioners hereby approves the agenda, with no changes.

WHEREAS, the Minutes of the December 22, 2009 meeting have been presented for review and that no changes have been made to the Minutes; BE IT RESOLVED, the Human Services Board accepts the Minutes as an accurate reflection of the content and actions taken at its previous meetings.

WHEREAS, the Director of Social Services has advised that all Income Maintenance case actions are in conformance with State, Federal, and County laws, rule, regulations and policies;
BE IT RESOLVED, that all Income Maintenance case openings, closings, suspensions, grant changes and any other action affecting client eligibility and payments documented in the Department on listing of proposed case actions, (through MAXIS computerized client files and reports, and MMIS II), Director's Actions, abstracts of payment, and reports of refunds and cancellations for the period 12/22/09 through 01/26/10 are hereby approved.
WHEREAS, the Directors of Public Health and Social Services have advised that all Public Health and Social Services case actions are in conformance with State, Federal and County laws rules, regulations and policies;
BE IT RESOLVED, that all Public Health and Social Services case openings, closings, payments, placements and licensing actions as documented in the Department on Agency listing for Licensing actions, case records, record of Social Service plan, purchase of service abstracts, reports of refunds and cancellations, payment authorizations and computer printouts for 12/22/09 through 01/26/10 are hereby approved.
WHEREAS, the Directors of Public Health and Social Services have advised that all personnel actions are in conformance with State and Federal law, Merit System rules, existing labor agreements and personnel policies;
BE IT RESOLVED, that all personnel actions for 12/22/09 through 01/26/10 for employees of the Departments of Public Health and Social Services as documented in the Department on proposed personnel actions listing, personnel abstracts, payroll reports and personnel actions forms are hereby approved.
WHEREAS, the Directors of Public Health and Social Services have advised that all Public Health claims and, Social Services and Welfare claims against the County submitted for payment for 12/22/09 through 01/26/10 have been reviewed and are eligible for payment pursuant to all applicable rules, regulations, laws and policies;
BE IT RESOLVED, that all bills as listed on listing of warrants and claims against the Public Health, Social Services and Welfare funds and warrant registers shall be approved as valid claims against the County and are hereby approved for payment.
WHEREAS, the Directors of Public Health and Social Services have advised that employee requests to attend conferences and/or training have met approval of Administration, and are within the budgetary guidelines of the Agency;
BE IT RESOLVED, that all requests presented by the Directors of Public Health and Social Services are hereby approved.

Whereas, the Department of Social Services has received an appeal of a fee determination of approximately $28,600 for the care of a young man in out-of-home care; and
Whereas, staff of the Department of Social Services has met with the family to re-evaluate the assessed fee; and
Whereas, the parent has provided information at this Board meeting and is agreeable to the reassessed fee amount;
Whereas, the Director of Social Services has recommended a fee that is based upon the 2008 income of the parent;
Now, therefore be it resolved the Wabasha County Human Services Board accepts hearing the appeal and to adjust the assessed fee based upon financial data provided. The correct monthly fee is $83.37. A total of $917.07 is due for the care of the child. A payment plan will be set between the Collections Officer and the client.

WINDHORST – DWELLE
Whereas, Wabasha County Human Services Board receives a number of documents requiring signature during the calendar year in the Public Health Services and Social Services Department; and
Whereas, the Minnesota Department of Health and the Minnesota Department of Human Services provide continuing funding for services through grants and allocation of funds authorized by Legislation, which also need signature by a representative of Public Health Services or Social Services staff; and
Whereas, these documents can be signed by administrative personnel as a matter of routine administrative procedure; and
Whereas, there has been a long history of administrative personnel signing certain documents on behalf of the Human Services Board; and
Whereas the authorized staff involved in these actions understand they will also comply with the Wabasha County Purchasing Policy, adopted Feb 5, 2002;
Now therefore be it resolved, the Wabasha County Board of Human Services hereby authorizes the Directors of Public Health and Social Services, or their designees, to approve purchases, sign contracts, placement agreements, and other legally binding documents on behalf of the County during 2010. There is a limitation of $2,500 per individual item and $12,000 per contract (excluding contract for medical assistance waivers). WINDHORST – RIESTER
Whereas, the Director of Social Services has provided detailed information to the Human Services Board on the vehicles available through the State bid contract; and
Whereas, a replacement vehicle has been included in the 2010 budget for Social Services; and Whereas, the 2007 Malibu is the vehicle to be moved to the general county car pool upon purchase of a 2010 vehicle; and
Whereas, the purchase of a vehicle by the Department of Social Services provides the highest level of reimbursement and is cost-effective;
Whereas, the Director of Social Services recommends the purchase of a 2010 vehicle and transfer of the 2007 vehicle to the general car pool; Now, therefore be it resolved, the Wabasha County Human Services Board approves the purchase of a 2010 Ford Fusion at a cost of $17,561.00, as bid by Tom Heffernan Ford of Lake City.

Whereas, the Department of Social Services has provided social services to the clients (identified below by case number) and have assessed a fee for the service provided; and
Whereas, Wabasha County has made diligent efforts to collect the fees due from each client unsuccessfully and these clients are now deceased; and
Whereas, the accounting & audit standards indicate the accounts should now be written off in accordance with standards adopted by the County;
Now therefore be it resolved, the Wabasha County Human Services Board reluctantly authorizes the Department of Social Services to write off the accounts as listed below:

Whereas, the Department of Social Services will have a Financial Worker vacancy with the retirement of Sandra Sanderson, effective February 5, 2010;
Whereas, the Human Services Board has approved the filling of this vacancy upon the request of Social Services staff; and
Whereas, the Social Services Director has recommended the hiring of Doris Ryan to make a lateral move to the Financial Worker position;
Now therefore be it resolved, the Wabasha County Board of Human Services hereby approves the hire of Doris Ryan as a Financial Worker in the Department of Social Services, effective February 16, 2010. There is no change in salary as Ms. Ryan will be accepting a lateral move within the Department. She will serve a 6-month probationary work period and her next Step date remains at December 26, 2010.

Whereas, Wabasha County Public Health Services has received donations of $360.34 from Greenfield Township Combined Drive and an additional $100 from Janice and Fred Timm;
Whereas the Director of Public Health Services has indicated these donations have been earmarked by the donors for the Special Needs Fund within PHS; and
Whereas, the Public Health Services Director recommends the Human Services Board formally accept the donations for the Special Needs Fund,
Now therefore be it resolved, the Wabasha County Board of Human Services hereby accepts the donated funds from the Greenfield Township Combined Drive and the Janice & Fred Timm family for use in the Special Needs Fund. The Board members express their thanks to the donors for their generous donations.

Whereas, Wabasha County Public Health Services has received a letter of resignation from Ms. Lois Armstrong, who has been a Home Health Aid for many years and is much appreciated by the residents she has served; and
Whereas the Director of Public Health Services recommends the Human Services Board accept this resignation;
Now therefore be it resolved, the Wabasha County Board of Human Services hereby accepts, with regrets, the letter of resignation from Ms. Lois Armstrong, effective February 10, 2010.

DWELLE – HALL
Whereas, Wabasha County Public Health Services has a Home Health Aid vacancy due to the resignation of Ms. Lois Armstrong; and
Whereas the Director of Public Health Services recommends filling the vacancy to maintain the current level of services to our county residents; and
Whereas a current part-time HHA may be selected to fill the full time position, and this part time position would then become available
Now therefore be it resolved, the Wabasha County Board of Human Services hereby authorizes the Director of Public Health Services to fill the upcoming full time vacancy. If it is filled with a current part time employee the Board hereby authorizes the Director to fill the part time Home Health position that was vacated.

HALL – WINDHORST
Whereas, Wabasha County Public Health Services has been offered an invitation by Mayo Health Systems to become a part of the Beacon Healthy Community grant application, and
Whereas this grant would provide funding for upgrades to the Public Health PH Documentation software that will allow electronic exchange, and interoperability, between health providers within the grant project. And provide funding for a quality improvement project around childhood asthma and adult diabetes, and
Whereas, Public Health Services needs to be in compliance with MN state law that requires electronic record interoperability by 2015, and
Whereas, the Public Health Director recommends that participation in this opportunity will provide a cost effective way to become interoperable with their records as well as improve the health status of children with asthma and adults with diabetes.
Now therefore be it resolved, the Wabasha County Board of Human Services hereby approves signing the letter of agreement to participate in this project with Mayo Health Systems.

DISCUSSION and INFORMATIONAL ITEMS

Environmental Health Services Report
Darrin Thompson was present to provide a short update on activity in the past month. His report included: End-of-year reporting to the State on septic systems, tank reporting, license “holds” for well contractors, septic ordinance update, and pipe grant activity regarding Lake Zumbro.
Social Services Quarterly Report
Marci Hitz, Supervisor shared information on the change in case count during the last quarter of 2009, highlights on out-of-home placements, and the activity of our mental health case managers.
4th Quarter Social Services Budget Report
Nicki Moline, Supervisor shared 2 reports on spending for calendar year 2009. The first report showed agency funding in all areas of the Department. There was a net gain of about $250,000 to the Department in 2009, resulting in a cash balance of approximately $2.7 million as we enter 2010. The second report highlighted the Court Services placement expenses. In 2009, the figures show $51,000 more in expenditures than were budgeted.
SHIP Grant Update
Judy Barton reviewed the 4 interventions that were chosen by the regional SHIP leadership team and submitted to the state. Now each county member of the team will be working with their local team of community partners to develop methods, for their county, to help community partners address these interventions and reach the goals of the SHIP grant. MDH is very prescriptive about where and what interventions can be chosen, so some of the local choices have been made for them by MDH
PHS/SS Miscellaneous
The meeting had 8 Winona State University nursing students in attendance. At various points during the meeting, efforts were made to more fully explain the items of discussion. The Board welcomed their attendance and thanked the students for their interest in County services. The students are completing a placement during their last semester at WSU. This group also has contact with our mental health clients and staff in Social Services.
Barton received a letter from a funding source that will bring $6,128 to the County as part of Federal stimulus funds. These funds will be used to purchase a regulation vaccine storage unit to store vaccines. They are currently using a normal kitchen refrigerator. It is a federal requirement by 2012 that all vaccines must be stored in a regulation vaccine storage unit.
Smith shared an update on transportation needs of residents in Lake City at the River Oaks Health Care Center. A reduction in the State access transportation funding has caused difficulty for the facility to get clients/residents to routine medical appointments.
Smith also provided a brief update on the Leadership Summit that was held on January 25th in St. Charles. 4 reports were given to the attendees from Environmental Services, Public Health, Highway Department, and Social Services. The reports highlighted coordination efforts between counties. State Representatives and Senators then shared their thoughts on the upcoming Legislative Session and how to best interact on a regular basis.

Being no further business to come before the Board, motion was made and duly carried at 10:50 a.m. to recess this meeting until 9:00 a.m. on February 23, 2010.
