Wabasha County Human Services Board

October 17th, 2017

The Human Services Board of Wabasha County, Minnesota, convened in Regular Session at the County Services Building, in the City of Wabasha, Minnesota at 8:00 AM October 17th, 2017 in accordance with MS 402.01 to 402.10.  The previous meeting was recessed to this time and place.  

The following members were present: Commissioners Hall, Goihl, Key, and Wobbe. Commissioner Springer was absent.

BE IT RESOLVED, that the Wabasha County Board of Commissioners hereby approves the agenda as presented.

WHEREAS, the Minutes of the September 19th regular board meeting have been presented for review and that no changes have been made to the Minutes;

BE IT RESOLVED, the Human Services Board accepts the Minutes as an accurate reflection of the content and actions taken at its previous meetings.

WHEREAS, the Director of Social Services has advised that all Income Maintenance case actions are in conformance with State, Federal, and County laws, rule, regulations and policies;

BE IT RESOLVED, that all Income Maintenance case openings, closings, suspensions, grant changes and any other action affecting client eligibility and payments documented in the Department on listing of proposed case actions, (through MAXIS computerized client files and reports, and MMIS II), Director's Actions, abstracts of payment, and reports of refunds and cancellations for the period 9/19/17 through 10/17/17 are hereby approved.

WHEREAS, the Directors of Public Health and Social Services have advised that all Public Health and Social Services case actions are in conformance with State, Federal and County laws rules, regulations and policies;

BE IT RESOLVED, that all Public Health and Social Services case openings, closings, payments, placements and licensing actions as documented in the Department on Agency listing for Licensing actions, case records, record of Social Service plan, purchase of service abstracts, reports of refunds and cancellations, payment authorizations and computer printouts for 9/19/17 through 10/17/17 are hereby approved.

WHEREAS, the Directors of Public Health and Social Services have advised that all personnel actions are in conformance with State and Federal law, Merit System rules, existing labor agreements and personnel policies;

BE IT RESOLVED, that all personnel actions for 9/19/17 through 10/17/17 for employees of the Departments of Public Health and Social Services as documented in the Department on proposed personnel actions listing, personnel abstracts, payroll reports and personnel actions forms are hereby approved.

WHEREAS, the Directors of Public Health and Social Services have advised that all Public Health claims and, Social Services and Welfare claims against the County submitted for payment for 9/19/17 through 10/17/17 have been reviewed and are eligible for payment pursuant to all applicable rules, regulations, laws and policies; 

BE IT RESOLVED, that all bills as listed on listing of warrants and claims against the Public Health, Social Services and Welfare funds and warrant registers shall be affirmed as valid claims against the County approved for payment.

WHEREAS, the Directors of Public Health and Social Services have advised that employee requests to attend conferences and/or training have met approval of Administration, and are within the budgetary guidelines of the Agency;

BE IT RESOLVED, that all requests presented by the Directors of Public Health and Social Services are hereby approved.

A.    Introduction of New Staff: Melissa Loechler RN was introduced to the Board.

ACTION ITEMS

Whereas, the Director of Public Health Services has reported a donation of $1,563.00 from the Wabasha Kellogg Area Community Foundation; and

Whereas, the Director of Public Health Services indicated these funds have been designated for the special needs fund in the Department; and

Whereas, the Director of Public Health Services recommends the Human Services Board accept the donation to be in compliance with County policy;

Now, therefore be it resolved, the Wabasha County Human Services Board accepts the donation of $1,563.00 from the Wabasha Kellogg Area Community Foundation, with said funds to be dedicated to the special needs fund within the Public Health Services Department.  The Human Services Board thanks the individuals for their generous donation to all agencies through the Local Giving Day.

Wobbe/Key

Whereas, Minnesota Statutes, section 256J.626, subdivision 4, requires counties to have a Biennial Service Agreement approved by the Minnesota Department of Human Services to receive consolidated funds for the Minnesota Family Investment Program (MFIP); and

Whereas the current MFIP Plan expires on December 31, 2017; and 

Whereas, the purpose of the agreement is to provide the Minnesota Department of Human Services with information about services and strategies to meet state established outcomes with the goal of increasing the economic stability of low income families served with consolidated funds; and

Whereas, the agreement will give the Minnesota Department of Human Services a comprehensive assessment of current efforts; insight into what types of assistance are needed; and information on new strategies to better serve program participants; and 

Whereas, the law requires the county to allow a period of no less than 30 days for public comments on the contents of the agreement; and

Whereas, the Director and Income Maintenance Supervisor recommend approval of the 2018-2019 County MFIP Biennial Plan;

Now, therefore be it resolved by the Wabasha County Human Services Board, the 2018-2019 County MFIP Biennial Plan is hereby approved, effective January 1, 2018 through December 31, 2019.

DISCUSSION and INFORMATIONAL ITEMS

A.    MOU with Fillmore County for IPI Visits

Tammy Fiedler discussed with the board new requirements for IPI visits and that counties must do a minimum of ten clinics per year.  Wabasha County has four clinics and the visits are for two clinics every other year at this time.  MDH is recommending that counties combine to complete visits.  Fillmore County will be completing our IPI visits. 

B.  SNAP Bonus Funds Update

John Dahlstrom discussed there may need to be some changes made to how some of the SNAP Bonus Funds will be spent.  A plan on how to use these funds was originally approved in July.  Some of the equipment costs were higher than anticipated and the EDMS reports are dependent on decisions made by the other participating counties and are still pending.  If changes are needed, it will be discussed at a future Human Services Board meeting.    

C.    SCHA Pay for Performance Change

Tammy Fiedler discussed Pay for Performance measures and that Wabasha County didn’t meet the 2nd measure.  Measure 2 was: Identified assessment needs are in the care plan. For seniors our Baseline was 84%, Target was 88% and we were at 87%.  For SNBC Baseline was 100%, Target was 95% and we achieved 93%.  Fiedler acknowledges this results in not getting $9,000.00 of Pay for Performance dollars.   The county did reach the

1st measure and received $6,000.00.

D.    MnCHOICES Referral Numbers

Tammy Fiedler discussed an increase in MnCHOICES referral requests and how this has impacted the HCBS team.  The average number in previous months was 8.  Recently the team has screened 21 new requests.  The impact is when short staffed, adding this 5 hour plus screening takes away from billable services resulting in decreased revenue.  In additional there is now a proposed cost share for 2018.  John Dahlstrom and Tammy Fiedler explained to the board the mechanism for funding these screenings called Random Moments.

E.    Human Services Advisory Committee Appointments and Updates

Tammy Fiedler and John Dahlstrom discussed the openings on HSB advisory committee. Two nurses recently went off the board because they were up for total years of services.  The board discussed and decided that would take applications for these vacant seats on the advisory board.  John and Tammy will work on an application and advertisement for Board approval.

PHS and SS MISCELLANEOUS:

Darrin Thompson was present for discussion regarding a county well.  Darrin will send a certified letter to the property owner letting them know the County intends to seal the well this winter, and they will need to find an alternative source of water for their property. 

Tammy Fiedler discussed the upcoming Project Community Connect event at St. Felix Gym on Friday October 20th from 10:00 am to 1:00 pm.

John Dahlstrom discussed the Social Services Budget.  If everything stays on target, out-of-home placement costs should be under budget.  He also discussed a potential legislative proposal for crisis bed funding to help alleviate emergency rooms and jails.  Rochester would be one of the regional centers and that counties would be asked for letters of support.
