Managed Care Plans

If you qualify for a Medical Assistance (MA), you may need to choose a health plan (see below) to receive health care services. Most plans will also ask you to choose a primary clinic.  Once you are approved for Medical Assistance, you may receive an enrollment form in the mail to select a plan.  You can either return the form to our office or contact us to let us know which plan you have chosen for the covered members in your household.

Exemptions from Managed Care
Medical Assistance recipients  are required to participate in managed care.  There are some exceptions.  Please contact us to find out who is exempt from enrolling in a managed care program.

Health Plans for Enrollees who are Children, Parents & Adults Without Children


Wabasha County has the following two health plans available if you are a child, a parent,  or an adult without children.

(1) Blue Plus 
Blue Advantage Families and Children from Blue Plus is a Minnesota Medical Assistance program (Minnesota’s state Medicaid program). This Minnesota Medical Assistance health care program is for families, children under age 21, pregnant women and adults under age 65 who receive Medical Assistance.

South Country Health Alliance’s Families and Children (also referred to as Medical Assistance or PMAP) is our largest program. Medical Assistance is health care coverage for children, families, pregnant women, and adults under age 65 that meet Minnesota income requirements.

South Country Health Alliance has several different programs for the disabled from which to choose:
  • SingleCare (SNBC)  Coverage for people with disabilities who are eligible for Medical Assistance but are not eligible for Medicare.
  • SharedCare (SNBC)  Coverage for people with disabilities who have SCHA Medical Assistance and Medicare through another health plan.
  • AbilityCare (HMO SNP)   Integrated coverage for people with disabilities that combines Medical Assistance and Medicare.
