Medicare Savings Programs

Medicare Savings Programs (MSP) help people who have low income pay the cost of their Medicare coverage. There are four different programs with specific rules for qualifying for each program.

Qualified Medicare Beneficiaries (QMB)
The Qualified Medicare Beneficiaries (QMB) program helps pay all Medicare costs including co-insurance and deductibles.

Service Limited Medicare Beneficiaries (SLMB)
The Services Limited Medicare Beneficiaries (SLMB) program pays only for the Medicare Part B premium.

Qualified Working Disabled Individual
The Qualified Working Disabled Individual (QWD) program pays only for the Medicare Part A premium.

Qualified Individuals (QI)
The Qualified Individuals  program pays only for the Medicare Part B premium.

How to apply for Medicare Savings Programs
Complete the Minnesota Health Care Programs Application for Certain Populations DHS-3876 (PDF) and return it to our office.  Or, if you cannot print out the application form, contact us and we can mail one out to you.  Or feel free to stop by our office to pick up an application.
